ABSTRACT

Basal cell and squamous cell carcinomas are the main neoplasms of lip and perioral location. We describe different techniques of successful surgical reconstruction, including flaps or simple incision and suture. Using the latter technique satisfactory results were demonstrated, although the incision made removed more than a third of the lip, contrary to the literature. Our goal is to emphasize that the common sense and experience of the surgeon should prevail in the choice of reconstructive method. Moreover, even if the priority is complete excision of the lesion, we cannot ignore the aesthetic and functional recovery objective.

ABSTRACT

Descreve-se a técnica para correção do entrópio, relatando-se o caso de paciente que apresentava essa alteração na pálpebra inferior esquerda, além de flacidez e pseudo-herniações de gordura nas regiões palpebrais direita e esquerda. Foram realizadas blefaroplastia superior e inferior bilaterais, e ressecção total em cunha do músculo orbicular e do tarso de ambas as pálpebras inferiores, seguida de reconstrução. A paciente evoluiu com bom resultado funcional e estético, livre de complicações.


This study describes a technique for correcting entropion based on the case of a female patient who had that alteration in the left lower eyelid, in addition to sagging and fat pseudo-herniation in the left and right eyelid areas. Bilateral upper and lower blepharoplasty was performed, with wedge resection of the orbicularis muscle and tarsus in both lower eyelids, followed by reconstruction. The patient presented good functional and aesthetic results, with no complications.

ABSTRACT

O carcinoma basocelular é o câncer de pele mais frequente. Quando recidivados, tumores desse tipo têm taxas de cura inferiores à dos tumores primários, razão pela qual é preconizado tratamento cirúrgico com re-excisão através de cirurgia micrográfica ou com controle de margens por congelação. Relata-se caso de carcinoma basocelular recidivado de grandes proporções que, após exérese total e correção por meio de retalho de rotação e enxerto de pele total, evoluiu com eclábio. Essa complicação inestética foi corrigida num segundo tempo através do retalho de Limberg. Apresenta-se alternativa para correção de eclábio e enfatiza-se a necessidade de incluir na formação do cirurgião dermatológico o aprendizado para a resolução de complicações.


Basal cell carcinomas are the most common type of skin cancer. Recurring basal cell carcinomas have lower cure rates than the primary tumors. Recurring tumors are usually treated with re-excision using micrographic surgery or by controlling the margins through freezing. A case of recurring basal cell carcinoma with large dimensions is reported. After total exeresis and correction using a rotation flap and total skin graft, the area developed eclabium, an unattractive complication that was corrected in a second surgery using a Limberg flap. This study presents an alternative correction for eclabium, and emphasizes the importance of training dermatologic surgeons how to resolve complications.
